How to prepare for ABA therapy intake
Here is what to have ready for the application, what to confirm after you apply, and what to do if something is missing.

What to have ready
Our application has 3 short steps. These are the details each step asks for.
About your child
- Full name and date of birth
- Diagnosis status
- Date on the evaluation report, or the scheduled evaluation date
- Pediatrician’s name
About you
- Your name and relationship to your child
- Phone and email
- Street address, city, state, and ZIP
Coverage and location
- Primary health insurance
- Member ID number
- Preferred clinic, or first available
- How you heard about us
Keep your child’s full evaluation report and insurance card nearby. The application asks for details from them. Please call to confirm how to share any documents.
What to confirm after you apply
An application does not start services. It also does not confirm coverage, an opening, or a start date. These are the topics to ask about, and you can call (737) 214-9205 with any of them.
| Topic | What to ask |
|---|---|
| Your contact | Who will be our contact, and how should we reach them? |
| Coverage | Does our plan cover ABA here? What does it require, and who handles each part? |
| Costs | What might we owe, and when will we know? |
| Paperwork | What do you need from us, and how should we share it? |
| Visit and assessment | Is a visit or assessment the next step, and who would we meet? |
| Openings | What is open at our preferred clinic or for in-home care? |
| Start date | What has to happen before therapy could begin? |
If something is missing
| If | Then |
|---|---|
| Your child’s evaluation is scheduled | Choose “Evaluation scheduled” and enter the date. |
| There is no diagnosis yet | Your plan may require a documented diagnosis before covering ABA. Read our diagnosis guide and call with your questions. |
| You are unsure about coverage details | Call intake if you are unsure how to answer a required field. |
| You cannot find your member ID | Check your insurance card, or call us at (737) 214-9205. |
| You have no clinic preference | Choose “First available.” |
| You would rather talk than type | Call us instead of using the application. |
Notes for your first conversation
These notes can make the first conversation easier.
- What your child loves, and how they ask for things
- What is hardest at home, and what you most want to change
- Days and times that would be difficult for your family
- Whether you are interested in clinic care, in-home care, or both
- Any preschool or other therapy already on your calendar
It also helps to ask who your contact will be during intake and what you should watch for next. Our question guide has more ideas.
