We use information to respond to families and referral sources, determine next steps for requested services, coordinate intake, process employment applications, communicate with you, protect the website from abuse, improve the site, and meet legal obligations.

Sensitive and health-related information

Please submit only information requested by a form. Health-related information is used only to respond to or coordinate the services you request. This website policy does not replace any Notice of Privacy Practices that may apply once a healthcare relationship is established. Do not use website forms for emergencies.

Optional website information assistant

When you choose to start the chat, the website loads an assistant provided by Chat Data. Chat Data and its AI model providers process the messages you send to generate answers from public website information. The assistant provides general website information and does not access your application, patient records, insurance eligibility, or appointments. Our website does not send your application fields or page context to the assistant. Chat Data’s handling of messages and technical information is described in its privacy policy. To improve website information, the assistant may look up a predefined topic and record its category, selected clinic, language category, and day. These topic records do not include your message, name, contact details, or a visitor or conversation identifier. We also count clicks on the assistant’s call and application links. Topic counts are not a complete record of conversations or a count of unique people. Our website management provider uses these counts to recommend information updates.

How we share information

We may share information with service providers that support hosting, forms, email, referral and intake systems, analytics, recruiting, and business operations, or when required by law. We do not sell or rent personal information.

Retention, security, and choices

We keep information only as long as reasonably needed for the purposes above and applicable healthcare, employment, and recordkeeping requirements.
